Information for Parents

New Permission Slip

In 2009, Student Life will switch to using a "blanket" permission slip. While this one form will provide everything we need for the entire year, it requires that you get the form notarized. If you do not have ready access to a notary, please let us know - we have at least one notary among the members of the church.

The new form is available as a PDF. You will need the Adobe Reader to open it. Some versions of the Adobe Reader will allow you to fill out the form on your computer, but you will need to print the form so it can be notarized.
