While this one form will provide everything we need for the entire year, it requires that you get the form notarized. The form must be renewed every 12 months or anytime the information it contains changes. The form is available as a PDF. You will need the Adobe Reader to open it. Some versions of the Adobe Reader will allow you to fill out the form on your computer, but you will need to print the form so it can be notarized.
